This is a fixed term post in elderly care available at Huddersfield Royal Infirmary and Calderdale Royal Hospital. The post holders will care for inpatients and outpatients based on the wards at HRI and/or CRH and in the newly developed outpatient unit.
The posts offer exciting opportunities to develop specialist skills in falls, stroke, community liaison, Parkinson’s, frailty, care homes and ortho-geriatrics and we would be happy to accommodate a range of other special interests as well.
Calderdale and Huddersfield NHS Foundation Trust have recently undergone a major Health and Social Care review involving commissioners, partner organisations, Local Authority and Primary Care. This has led to the exciting programme of developing “Care Closer to Home” with specialist hospital care being delivered at or near the patient’s home. Examples include Quest for Quality, a newly commissioned service in Calderdale. This is a geriatrician lead specialist service for residential and nursing homes in Calderdale. The geriatrician leads a team of community based matrons working in residential care to improve the medical care of patients and reduce hospital admissions. The team takes an active role in managing acute emergencies and spends a significant part of their time in a proactive prevention role, for example, advanced care planning or medication reviews. The geriatrician reviews patients where necessary and supports a weekly MDT. Care Closer to Home will involve further community based geriatric services being developed and this is an exciting opportunity to lead the development of these services.
In addition to a greater community based role for hospital specialists there are proposals for the Trust to undergo significant internal reconfiguration so that all acute hospitals services would be based on one site. This would allow for further development of inpatient services for example, an acute frailty service and Hospital without Walls.
Applicants must be on the General Medical Council Register and must continue to hold a Licence to Practise.
You must have appropriate UK professional registration.
