We have an excellent opportunity for a highly motivated physiotherapists or occupational therapist to join the expanding Frailty team.

You will be part of a friendly, supportive, and professional integrated team comprising of Consultants, Nurses, occupational therapists, physiotherapist, co-ordinators, advanced clinical practitioners, pharmacists, social services and community services.

High quality patient care is at the forefront of our decision making and we are enthusiastic about welcoming new ideas enabling the successful candidate to take an active role in service development and leadership.

The role focuses on meeting the needs of the Frail population, requiring complex assessment and decision making to provide the right care in the right place within the first 72 hours of the frail patient’s admission, with a focused patient centred approach to admission avoidance and reduction in length of stay. You will be based at the front end of the hospital, working collaboratively to start the assessment process from the moment the patient arrives in the emergency department or our assessment wards.

You will be joining a dynamic, challenging team which provides an excellent high standard of care to frail patients, their carers, families and support networks. The Frailty Team is continually developing and progressing to meet best practice guidelines.
